Usage and dosage
Adults: intramuscular injection of 2 ~ 4G (2 ~ 4 branches) a day, administered four times; The dosage of intravenous drip or injection is 4 ~ 8g (4 ~ 8 branches) a day, which is administered in 2 ~ 4 times. The daily dose of patients with severe infection can be increased to 12g (12 tubes), and the maximum daily dose is 14g (14 tubes).
Children: intramuscular injection, 50 ~ 100mg / kg per day, divided into 4 times; Intravenous drip or injection is given 2 ~ 4 times a day according to the body weight of 100 ~ 200mg / kg. The maximum daily dose is 300 mg / kg according to body weight.
Term newborn: once every 12 hours on the first and second day of birth, once every 8 hours on the third day to two weeks, and once every 6 hours thereafter.
Preterm infants: 12.5 ~ 50mg / kg at the first week, 1 ~ 4 weeks and over 4 weeks of birth, once every 12 hours, 8 hours and 6 hours respectively.
For patients with renal insufficiency: when the creatinine clearance rate is 10 ~ 50ml / min or less than 10ml / min, the administration interval should be extended to 6 ~ 12h and 12 ~ 24h respectively.
The higher the concentration of ampicillin sodium solution, the worse the stability. At 5 ºC, 1% ampicillin sodium solution can maintain its biological titer for 7 days, but 5% solution is 24 hours. Ampicillin sodium intravenous drip with a concentration of 30mg / ml can maintain at least 90% of its titer at room temperature for 2 ~ 8 hours, and 90% of its titer can be maintained in the refrigerator for 72 hours. The stability can be reduced by the presence of glucose, fructose and lactic acid, and also decreased with the increase of temperature.
For intramuscular injection, 0.9 ~ 1.2ml, 1.2 ~ 1.8ml and 2.4 ~ 7.4ml sterile injection water of 125mg, 500mg and 1g ampicillin sodium can be dissolved respectively. The concentration of ampicillin sodium intravenous drip should not exceed 30mg / ml.
Adverse reaction
The adverse reactions of this product are similar to penicillin, and allergic reactions are more common. Rash is the most common reaction, mostly occurring 5 days after treatment, showing urticaria or maculopapular rash; Interstitial nephritis can also occur; Anaphylactic shock is occasionally seen. Once it occurs, it must be rescued on site, and treatment measures such as keeping airway unblocked, oxygen inhalation and administration of adrenaline and glucocorticoid must be taken.
Granulocytopenia and thrombocytopenia are occasionally seen in patients treated with ampicillin. Antibiotic associated enteritis is rare, and a few patients have elevated serum aminotransferase. Intravenous administration of high-dose ampicillin can cause neurological toxic symptoms such as convulsions. Infants with ampicillin can have increased intracranial pressure, which is manifested as anterior halogenation
