Sulfonamides are broad-spectrum antibacterial drugs, but because many common clinical pathogens are resistant to these drugs, they are only used for infections caused by sensitive bacteria and other sensitive pathogenic microorganisms. Indications for sulfadiazine (excluding the combination of this drug and trimethoprim) are as follows: 1. Treatment and prevention of epidemic cerebrospinal meningitis caused by sensitive meningococci. 2. In combination with trimethoprim, it can treat otitis media and skin and soft tissue infections caused by Haemophilus influenzae, Streptococcus pneumoniae and other streptococci which are sensitive to it. 3. Astrocardiosis. 4. Adjuvant therapy for chloroquine-resistant falciparum malaria. 5. It is a secondary drug for the treatment of cervicitis and urethritis caused by Chlamydia trachomatis. 6. A secondary agent for the treatment of neonatal conjunctivitis including somatoconjunctivitis caused by Chlamydia trachomatis.
1. For the treatment of general infected adults, the usual dosage is taken orally, 1g once, twice a day, and the first dose is doubled. For infants and children over 2 months of age, take orally 25-30mg /kg twice a day according to body weight, double the first dose (the total dose should not exceed 2g). 2. For prevention of epidemic cerebrospinal meningitis in adults, the usual dosage is taken orally, 1g once, twice a day, for 2 days. For infants and children over 2 months old, 0.5g daily for 2-3 days.
1. Allergic reactions are common and can be manifested as drug eruptions. In severe cases, exudative erythema multiforme, exfoliative dermatitis and epidermolysis atrophic dermatitis bullosa can occur. There are also manifestations of photosensitive reaction, drug fever, joint and muscle pain, fever and other serum disease-like reactions. 2. Neutropenia or deficiency, thrombocytopenia and aplastic anemia. Patients may present with sore throat, fever, pallor, and a tendency to bleed. 3. Hemolytic anemia and hemoglobinuria. The deficiency of glucose 6-phosphate dehydrogenase is more common in neonates and children than in adults. 4. Hyperbilirubinemia and neonatal kernicterus. Because sulfanilamide and bilirubin compete for protein binding site. Can cause the increase of free bilirubin. Neonatal liver function is not perfect, it is easier to produce hyperbilirubinemia and neonatal jaundice, occasionally can produce nuclear jaundice. .5. Liver damage. Can occur jaundice, liver dysfunction, severe cases can occur acute liver necrosis. 6. Kidney damage. Crystalline urine, hematuria and tubular urine may occur. Occasionally, interstitial nephritis or tubular necrosis and other serious adverse reactions occurred. 7. Nausea, vomiting, hypogastric absorption, diarrhea, headache, fatigue, etc., generally mild symptoms, do not affect the continuation of medication. Occasionally, patients develop Clostridium difficile enteritis and need to stop the drug. .8. Goiter and hypofunction occasionally occur. 9. Central nervous system toxic reactions may occasionally occur in the form of psychosis, disorientation, hallucinations, euphoria or depression. Once appear all need to stop medicine immediately. Serious adverse reactions caused by this product, although rare, can be fatal, such as exudative erythema multiforme, exfoliative dermatitis, epidermolysis atrophic dermatitis bullosa, fulminant liver necrosis, agranulocytosis, aplastic anemia and other hematological abnormalities. Treatment should be closely monitored and should be stopped when rash or other early signs of reaction appear.
[Contraindicated] 1. Contraindicated for those who are allergic to sulfa drugs. 2. Pregnant and lactating women are prohibited. 3. It is forbidden for infants under 2 months of age. 4. It is forbidden for those with poor liver and renal function.
1. The following conditions should be used with caution: glucose-6-phosphate dehydrogenase deficiency, hemoporphyria, water loss, shock, and elderly patients. 2. Cross allergic reactions. Patients who are allergic to one sulfa drug may be allergic to other sulfa drugs. 3. Patients who are allergic to furosemide, sulfoxide, thiazide diuretics, sulfonylureas, carbonic anhydrase inhibitors may also be allergic to sulfonamides. 4. Drink plenty of water each time you take this product. Adequate water intake should also be maintained during administration, so that the daily urine output of adults is maintained at least 1200ml. If the application of this product course of treatment is long, when the dose is large, in addition to drinking more water should be taken with sodium bicarbonate. 5. Examinations should be paid attention to during treatment: 1. Total blood examination is particularly important for patients who have received a longer course of treatment. 2. Regular urine examination (routine urine examination every 2 to 3 days) during treatment to detect crystalline urine that may occur during long course or high dose treatment. 3. Liver and renal function tests. 6. Blood concentration should be measured in severely infected patients. For most infectious diseases, the concentration of free sulfa is 50 ~ 150μ g/ml (120 ~ 150&mu for severe infection; g/ml) can be effective. Total sulfonamide blood concentration should not exceed 200μ g/ml, if more than this concentration, the incidence of adverse reactions increased. 7. Due to the low solubility of this product in urine, the chance of crystalline urine will increase. Therefore, it is generally not recommended for the treatment of urinary tract infection. 8. Do not arbitrarily increase the dose, increase the number of medication or extend the course of treatment to prevent accumulation and poisoning. 9. Since this product can inhibit the growth of Escherichia coli and hinder the synthesis of B vitamins in the intestine, those who use this product for more than a week should be given B vitamin to prevent its deficiency.
