[Properties] This product is white powder or crystalline powder.



This product is only suitable for the treatment of penicillase-producing staphylococcal infections, including sepsis, endocarditis, pneumonia and skin and soft tissue infections. Can also be used for pyogenic streptococcus or pneumococcus and penicillin-resistant staphylococcus caused by mixed infection.



[Specification and Model] 0.5g



[Usage and Dosage] For intramuscular injection of this product, add 2.8ml of sterilized water for injection for every 0.5g. Intramuscular injection of 4~6g per day for adults, divided into 4 times; Intravenous infusion of 4 to 8g per day for adults, divided into 2 to 4 times. The daily dose for severe infections can be increased to 12g. Children weighing less than 40kg were given 12.5-25mg /kg every 8 hours according to their body weight, and adult dose was given to those weighing more than 40kg. The neonatal weight was less than 2kg, and the neonatal weight was 25mg/kg every 12 hours for those aged 1 to 14 days, and 25mg/kg every 8 hours for those aged 15 to 30 days. Weight over 2kg: 25mg/kg every 8 hours for those aged 1 to 14 days, 25mg/kg every 6 hours for those aged 15 to 30 days. Patients with mild and moderate renal hypofunction do not need to adjust the dose, and patients with severe renal hypofunction should avoid the use of large dose to prevent the occurrence of toxic reaction of central nervous system.



[Adverse Reactions] 1. Allergic reactions: urticaria and other types of rash are common, leukopenia, interstitial nephritis, asthma attacks and serum disease type reactions are rare; Anaphylactic shock is occasionally seen. Once it occurs, emergency treatment must be carried out on the spot, such as keeping the airway smooth, inhaling oxygen, using adrenal cord and glucocorticoids. 2. Intravenous use of this product can produce nausea, vomiting and serum aminotransferase elevation. 3. Large dose intravenous infusion of this product can cause convulsions and other toxic reactions in the central nervous system. 4. Hematuria, proteinuria and uremia have been reported in infants after using large doses of this product.



It is contraindicated in patients with penicillin allergy history or penicillin skin test positive.

People who are allergic to one kind of penicillin may be allergic to other penicillins and penicillamine. About 5%-7% of people who have a history of penicillin-induced anaphylactic shock may have cross-allergy to cephalosporins. 3. Patients with asthma, eczema, hay fever, urticaria and other allergic diseases and liver diseases should use this product with caution.

This product is incompatible with aminoglycosides, norepinephrine, m-hydroxylamine, phenobarbital, vitamin B group, vitamin C and other drugs, so it is not suitable to be instilled in the same bottle. 2. Prosulfonate can reduce the secretion of the renal tubule of phenoxacillin and prolong the serum half-life of this product. 3. Aspirin and sulfonamides can inhibit the binding of this product to serum protein and improve the free blood drug concentration of this product.



[Drug overdose] The main manifestation of drug overdose is the adverse reaction of the central nervous system, so the drug should be stopped in time and given symptomatic and supportive treatment. Hemodialysis did not clear oxacillin.



Pharmacology: This product is acid - and penicillin-resistant penicillin. The antimicrobial activity of phenoxacillin against penicillase-producing staphylococcus was lower than that of penicillase-producing staphylococcus. Oxacillin plays a bactericidal role by inhibiting bacterial cell wall synthesis. Toxicology: Not clear.



[Pharmacokinetics] Phenoxacillin is acid-resistant and can absorb 30%~33% of the dose administered by oral administration. After intramuscular injection of oxacillin 0.5g, the peak plasma concentration (Cmax) reached 16.7mg/L in 0.5 h. Double the dose. Blood concentration also doubled. The plasma concentration was 9.7mg/L at the end of intravenous infusion and 0.16mg/L 2 hours later. The peak plasma concentration was 51.5mg/L and 47.0mg/L after intramuscular injection of 20mg/kg in neonates 8 to 15 days and 20 to 21 days of birth, respectively. The binding rate of benzacillin protein was 93%, which could reach the effective therapeutic concentration in liver, kidney, intestine, spleen, pleural effusion and articular cavity fluid, while the concentration in ascites and sputum was low. It is difficult for oxacillin to cross the normal blood-brain barrier and can enter the fetus through the placenta. A small amount of oxacillin is secreted into the milk. The elimination half-life of this product for healthy adults is 0.4~0.7 hours; The elimination half-lives of infants 8 to 15 days old and 20 to 21 days old were 1.6 days and 1.2 days respectively. About 49% of oxacillin is metabolized in liver, about 40% is excreted in urine as original drug after intramuscular injection, and about 10% is excreted through biliary tract. Neither hemodialysis nor peritoneal dialysis can clear this product.
