Item character: This product is sugar coated tablets or film coated tablets. After removing the coating, it appears white or slightly yellowish green.

Indication:
1. Typhoid and other Salmonella infections: the drug is selected for typhoid and paratyphoid caused by sensitive strains. This product is generally not suitable for gastroenteritis infected by Salmonella. It can still be selected if the condition is serious and may be complicated with sepsis; Fluoroquinolones are the first choice in adult typhoid and paratyphoid salmonella infection (they should not be used by pregnant women and children).
2. Haemophilus influenzae type b meningitis resistant to ampicillin or Streptococcus pneumoniae, Neisseria meningitidis meningitis and sensitive gram-negative bacilli meningitis in patients allergic to penicillin. This product can be used as one of the selected drugs.
3. Brain abscess, especially otogenic, is often a mixed infection of aerobic and anaerobic bacteria.
4. Severe anaerobic infection, such as infection caused by Bacteroides fragilis, is especially suitable for those whose lesions involve the central nervous system. It can be combined with aminoglycoside antibiotics to treat abdominal infection and pelvic infection, so as to control the simultaneous aerobic and anaerobic infection.
5. When there are no other low toxicity antibiotics to replace, it is often used with aminoglycosides to treat various serious infections caused by sensitive bacteria, such as sepsis and lung infection caused by Haemophilus influenzae, Salmonella and other gram-negative bacteria.

Usage and dosage:Oral.
1. Adults take 1.5 ~ 3G a day, 3 ~ 4 times.
2. Children should take it three to four times according to their body weight of 25 ~ 50mg / kg a day.
3. Newborns shall not exceed 25mg / kg a day and shall be taken four times.

Adverse reactions
1. The toxic reaction to hematopoietic system is the most serious adverse reaction of chloramphenicol. There are two different forms.
(1) Dose related reversible bone marrow suppression is common in patients with blood drug concentration of more than 25mg / L. The clinical manifestation is anemia and can be accompanied by leucopenia and thrombocytopenia.
(2) Bone marrow toxicity unrelated to dose is often manifested as severe and irreversible aplastic anemia. Patients with aplastic anemia can have an incubation period of several weeks to months, which is not easy to be found early. Its clinical manifestations include bleeding tendency caused by thrombocytopenia, such as ecchymosis, ecchymosis and epistaxis, as well as signs of infection caused by granulocytopenia, such as high fever, sore throat Jaundice, etc. Most aplastic anemia occurs after oral administration of chloramphenicol.
2. Hemolytic anemia can occur in some patients with congenital deficiency of glucose-6-phosphate dehydrogenase.
3. Grey baby syndrome. The typical case occurs within 48 hours after birth. High dose chloramphenicol is administered. Grey baby syndrome can occur after treatment for 3 ~ 4 days, and the blood drug concentration can be as high as 40 ~ 200mg / L. The clinical manifestations were abdominal distension, vomiting, progressive pallor, cyanosis, microcirculation disturbance, Non rising body temperature and irregular breathing. It often occurs when large doses of chloramphenicol (more than 25mg / kg per day according to body weight) are applied to preterm infants or newborns, and similar manifestations can also occur when larger doses (about 100mg / kg per day according to body weight) are applied to adults or older children. If the drug was stopped early, it could recover completely.
4. Long term treatment with this product can induce bleeding tendency, which may be related to bone marrow suppression, obstruction of vitamin K synthesis caused by reduction of intestinal flora, prolongation of prothrombin time, etc.
5. Peripheral neuritis and optic neuritis often occur during long-term treatment. Drug withdrawal as soon as possible is often reversible. There are also cases of optic nerve atrophy and blindness.
6. Gastrointestinal reactions, including diarrhea, nausea, vomiting, etc.
7. Allergic reactions are rare. It can cause various rashes, solar dermatitis and angioneuroedema. It is generally mild and can improve rapidly after stopping the drug.
8. Double infection can cause lung, gastrointestinal and urinary tract infections of Proteus, Pseudomonas aeruginosa, Staphylococcus aureus and fungi.

Matters needing attention:
1. Since irreversible bone marrow suppression may occur, this product should be avoided from repeated treatment.
2. Patients with liver and kidney impairment should avoid using this product. If it must be used, reduce the application and monitor the blood drug concentration, so that the peak concentration (Cmax) is below 25mg / L and the valley concentration is below 5mg / L. If the blood drug concentration exceeds this range, it can increase the risk of bone marrow suppression.
3. When taking this product orally, you should drink enough water and take it on an empty stomach, that is, take it 1 hour before meal or 2 hours after meal, in order to reach the effective blood drug concentration.
4. The peripheral hemogram should be checked regularly during the treatment, and the reticulocyte count should be checked for long-term treatment, and bone marrow examination should be performed if necessary, so as to timely find the reversible bone marrow suppression related to the dose, but the whole hemogram examination can not predict the aplastic anemia that usually occurs after the completion of treatment.
5. Interference to diagnosis: when urine sugar is measured by copper sulfate method, patients with chloramphenicol can produce false positive reaction.

Medication for pregnant and lactating women:
Because chloramphenicol can penetrate the placental barrier, it may cause toxic reactions to preterm infants and full-term newborns, resulting in "grey infant syndrome". Therefore, it is not suitable to use this product during pregnancy, especially at the end of pregnancy or delivery.
This product is secreted from milk and may cause adverse reactions in lactating infants, including severe bone marrow suppression. Therefore, this product is not suitable for lactating women. When it must be used, lactation should be suspended.
