Functional Indications
This product is suitable for infection caused by streptococcus, Staphylococcus, anaerobic bacteria and other sensitive strains.
[Usage and dosage] Adults, 0.15 ~ 0.3g a time, 4 times a day oral, severe infection can be increased to 0.45g a time, 4 times a day oral. 4 weeks or more children, according to the body weight of 8 to 16mg/kg, divided into 3 to 4 times a day oral.
[Adverse reaction] 1. Gastrointestinal reactions: including nausea, vomiting, abdominal pain, diarrhea and other diseases; In severe cases, there are abdominal cramps, abdominal tenderness, severe diarrhea (watery or pus and blood), accompanied by fever, abnormal thirst, and fatigue (pseudomembranous enteritis). Diarrhea, enteritis and pseudomembranous enteritis may occur during treatment or after drug withdrawal. 2. Allergic reactions: Usually mild to moderate measles-like rash is most common, followed by blister rash and urticaria, occasionally erythema multiforme, exfoliative dermatitis, and some manifestations of Stevens-Johnson syndrome. 3. Abnormal liver function, abnormal kidney function, occasionally neutropenia and eosinophilia.
Contraindication Contraindicated in patients with a history of allergy to clindamycin and lincomycin.
[Precautions] 1. Patients who are allergic to lincomycin may also be allergic to clindamycin. 2. Interference with diagnosis: Serum alanine aminotransferase and aspartate aminotransferase may increase after medication. 3. It should be used with caution in the following cases: (1) bowel disease or a history of bowel disease, especially ulcerative colitis, crohn's disease or antibiotic-associated enteritis. (2) decreased liver function. (3) Severe renal function decline. 4. To prevent the occurrence of acute rheumatic fever, clindamycin should be used to treat hemolytic streptococcal infection for at least 10 days. 5. During the medication, pay close attention to the number of stool, such as increased defecation times, should pay attention to the possibility of pseudomembranous enteritis, must be stopped in time and appropriate treatment. 6. Treatment of pseudomembranous enteritis caused by clindamycin is expected to be effective in mild cases, while water and electrolyte disturbances need to be corrected in moderate to severe cases. If the condition does not improve significantly after the above treatment, metronidazole should be given orally 250 ~ 500mg, 3 times a day. If relapse, metronidazole can be used orally. If still ineffective, vancomycin (or norvancomycin) can be used orally. Adults should be given 0.5-2.0 g a day, divided into 3 to 4 times. 7. Generally, the dosage of this product does not need to be reduced in patients with renal dysfunction (except those with severe renal dysfunction). Patients with severe renal function and/or decreased liver function should be monitored for serum drug concentration when taking this product. 8. This product can cause excessive proliferation of microorganisms that are not sensitive to this product, especially fungi, so it is necessary to pay attention to the occurrence of double infection when applying. Precautions during pregnancy and lactation:
Animal experiments have shown that this product has no effect on the fetus, but its application in human pregnant women is still inexperienced, so it should be used with caution in pregnant women. Notes for the elderly:
Clinical studies of this product do not have enough data on the elderly to determine whether the elderly respond differently to this drug than the young. There was no clinically significant difference in pharmacokinetics between young and old volunteers with normal liver and renal function who were given clindamycin orally or intravenously. Diarrhea and antibiotic-associated colitis have been reported to be more common and severe in the elderly. Therefore, the elderly patients with diarrhea should be closely monitored.
Drug interaction 1. Neuromuscular blockages that can enhance inhaled anesthetics, leading to skeletal muscle weakness and respiratory depression or paralysis (apnea), should be noted during or during surgery. Treatment with anticholinesterase drugs or calcium salts is expected to be effective. 2. Used with antiperistaltic antidiarrheals and antidiarrheals containing white clay: Clindamycin has the potential to cause pseudomemoral enteritis with severe watery diarrhea during and even weeks after treatment. Because it can delay the discharge of colonic endotoxin, which leads to prolonged and aggravated diarrhea, anti-peristalsis and antidiarytic drugs should not be used together. Antidiarrheal drugs containing white clay and clindamycin are taken orally at the same time, the absorption of the latter will be significantly reduced, so the two should not be taken at the same time, and should be taken at a certain interval (at least 2 hours). 3. This product has a neuromuscular blocking effect and when combined with antimyasthenics will result in the latter's effect on skeletal muscle
