The main ingredient of this product is ethambutol hydrochloride.
Indications This product is suitable for the combined treatment of tuberculosis and extrapulmonary tuberculosis caused by Mycobacterium tuberculosis with other anti-tuberculosis drugs, and also for the treatment of atypical mycobacterium tuberculosis infection.
1. Common optic nerve damage, such as retrobulbar optic neuritis, optic nerve center fiber damage.
(1) It may be related to the reduction of the content of copper, zinc and other metal elements caused by chelation of this product. The incidence of retrobulbous optic neuritis is about 0.8%, which is related to the dose and course of treatment. Long-term medication is easy to occur when the daily dose is greater than 25mg/kg. The incidence of 15mg/kg daily dose is 1%, 25mg/kg is 6%, and 35mg/kg is increased to 15%.
(2) Presents with blurred vision, eye pain, red-green color blindness or vision loss, reduced field of vision. If the reaction is detected early and the drug is stopped in time, it can disappear within weeks or months. Permanent loss of visual function rarely occurs.
2. Rare chills, joint swelling and pain (especially the big toe, condyle, and knee joint) and skin fever and tension on the surface of the diseased joint (acute gout, hyperuricemia).
3. Gastrointestinal discomfort, nausea, vomiting, diarrhea, liver damage, peripheral neuritis (often manifested as numbness, tingling, burning pain or weakness of hands and feet) and allergic reactions (often manifested as rash, itching, headache, fever, joint pain), etc.
Contraindications Caution should be exercised in persons allergic to this product, known optic neuritis, alcohol intoxication, and persons under the age of 13 years.
Precautions 1. Use with caution in patients with gout, optic neuritis, diabetic fundus lesions, hepatic and renal dysfunction. Patients with decreased renal function should reduce dosage.
2. The bacteria can rapidly develop resistance to this drug when used alone, so it must be used in combination with other anti-tuberculosis drugs.
3. Check during treatment:
(1) Eyes, visual field, visual acuity, red and green discrimination, etc., should be checked once a month before and during the course of medication, especially for patients with a long course of medication and a daily dose of more than 15mg/kg.
(2) Because this product can increase the concentration of uric acid in the blood and cause gout attacks, serum uric acid should be measured regularly during the course of treatment.
4. Interference to diagnosis, taking this product can increase the determination value of blood uric acid concentration.
5. If gastrointestinal irritation occurs, this product can be taken with food. The effective blood concentration may not be reached by taking the daily dose in different doses, so the product should be taken once a day.
1. Since this product can cross the placenta and the fetal blood concentration is about 30% of the maternal blood concentration, animal experiments have shown that this product can cause deformability. Although no problem has been confirmed in humans, pregnant women should still constrict this product.
2. This product can be secreted into the milk, and the concentration is similar to that of the mother's blood drug. Therefore, lactating women should not use this product.
Drug use in children 1.Clinical data are not available in children under 13 years of age, and it is not easy to monitor visual changes in children, so this product should not be used in children under 13 years of age;
2.The dosage for children over 13 years old is the same as that for adults.

Drug interactions 1. Aluminum salts, including DDI buffers, reduce the absorption of this product.
2. The combination of this product and verapamil can reduce the absorption of the latter.
3. Combined with neurotoxic drugs can increase the neurotoxicity of this product, such as optic neuritis or peripheral neuritis.
4. Combined with ethylthioisonicotinamine can increase jaundice hepatitis, optic neuritis and other adverse reactions.
Drug overdose Treatment of drug overdose:
1. Drug withdrawal.
2. Symptomatic treatment:
(1) retrobulbar optic neuritis can be used vitamin B6, multivitamin and zinc copper preparation.
(2) To restore vision, you can choose dexamethasone 5mg, daily intravenous drip or injection after the ball; Tolasurin 12.5mg, daily retrobulbar injection; Cortisone chloride 200mg daily. It can also be taken orally 20mg prednisone, 2-3 times a day. At the same time give vitamins and so on. During convalescence, acupuncture treatment, oral dibazole, niacin, or daily intramuscular injection of placental tissue fluid can be given.
(3) Hemodialysis and peritoneal dialysis if necessary.

(1) In large doses, this product can cause cleft palate, brain exposure and spinal deformity in mice;
(2) Mild cervical deformity in rats;
(3) Cyclops, short limbs and cleft palate were observed in rabbits.
Pharmacokinetics Absorption of 75% ~ 80% through gastrointestinal tract after oral administration. Widely distributed throughout the body in tissues and body fluids (except cerebrospinal fluid).

3. It is mainly metabolized by the liver, and about 15% of the dose is metabolized into inactive metabolites. Excreted by glomerular filtration and renal tubule secretion; About 80% is excreted within 24 hours of administration, at least 50% is excreted in its original form, and about 15% is inactive metabolites. About 20% is excreted in the stool in its original form. The concentration of drugs in milk is about equal to that in maternal blood. Considerable amounts of ethambutol can be cleared from the body by hemodialysis and peritoneal dialysis.
